My dad was diagnosed with metastatic kidney cancer and he is overseas

Hi all. 
this is my first time ever posting in an online forum… but I feel so utterly hopeless and distraught…

my dad, who is 61 and was the picture of perfect health was diagnosed with metastatic kidney cancer 5-months ago. 
He is overseas…. And it has been a challenge for him to get the medicines he requires through the public system… we can’t afford private care… 

He was told his medication (cabozantinib) had been discontinued from the public hospital… and just like that he has gone 4 days without any treatment. 
He has now developed extremely severe new pain on his left upper back (around the shoulder blade), which is making it difficult to breath, move, talk….. 

I feel so hopeless… it feels like there’s nothing I can do…. He was told he has neuritis and given neurofen and tramadol… but he is still in severe excruciating pain…. What can I do? 
What sort of medications can he have to manage that pain?? Please give me any advice… any advice would be appreciated…
