Transferring mother interstate from WA to NSW

Hi, I am looking to move my mother from Fremantle WA to Orange NSW to continue treatment with better family support. Just wondering if anyone here has done somthing similar and if anyone knows of any doctors in Orange to whom I can contact to discuss transfer proceedures? Thanks in advance...
