Anyone have any tips for breakthrough pain

Does anyone have any advice for helping with breakthrough back pain in pancreatic cancer. 
my otherwise very healthy husband is terminal but it is early days. His pain is under control except for middle of the night! Not every night but a lot. Has anyone found a reason a sleeping position , anything that helps with this?

Has there been any mention of a nerve block for his pain, unfortunately it isn’t always successful .
